Welcome to the High Five Rewards Program – Where Your Loyalty Gets Rewarded!

The Supply Doc High 5 Rewards Program is designed to reward consistent purchasing behavior and strengthen long-term partnerships with our valued clients. Eligible participants who place qualifying dental supply orders for three consecutive months will receive a credit equal to 20% of their average spend during those three months, applied in the fourth month.

After completing 3 consecutive months of purchases, you'll receive a credit equal to 20% of your average monthly spend.

Eligibility Requirements:

To qualify for the High 5 Rewards Program, clients must meet the following criteria:

1. Consecutive Orders: Place one qualifying order each month for three consecutive months.

2. Minimum Order Value: Each monthly order must be a minimum of $999 (before taxes and shipping).

3. Account Status: The client account must be in good standing with no past due balances at any point during the program period. 

Product Scope:

• The program applies only to dental supply purchases.
• Equipment, service, or repair orders do not count toward eligibility or rebate calculation.

Reward Calculation:

• Upon completion of the third consecutive qualifying order, the client will receive a credit equal to 20% of the average spend across the three previous months.
• This credit will be applied to the client’s account and may be used for future dental supply purchases.

Terms and Conditions:

• Program is subject to change or termination at any time by Supply Doc.
• Credit must be used within 90 days of issuance and is non-transferable.
• Only one High 5 Reward credit may be earned per client within any rolling 4-month cycle.
• All disputes or exceptions must be reviewed and approved by Supply Doc management.
